摘要:目前血管内介入治疗颅内动脉瘤已广泛应用于临床。相较于开颅夹闭手术,血管内介入治疗具有创伤小、恢复快、风险低的优点。但颅内复杂性动脉瘤,尤其是分叉部宽颈动脉瘤(WNBA)的血管内介入治疗仍是目前的治疗难点之一。由于WNBA特殊的解剖结构,传统的血管内栓塞治疗难以充分填塞瘤腔与保障流出道的通畅。随着介入技术的发展,以及设备如类支架装置、血管内置物和囊内扰流装置等的开发与更新,使WNBA也能得到有效治疗。本文通过介绍新的介入技术和设备等内容,阐述血管内介入治疗WNBA的现状与进展。
Abstract:At present, endovascular intervention therapy for intracranial aneurysm has been widely used in clinic. Compared with craniotomy, endovascular intervention therapy has the advantages of less trauma, faster recovery, and lower risk. However, endovascular intervention therapy for intracranial complex aneurysms, especially wide-neck bifurcation aneurysm (WNBA), is still one of the difficulties in the treatment. Due to the special anatomical structure of WNBA, traditional endovascular embolization therapy is difficult to fully fill the tumor cavity and ensure the patency of the outflow tract. With the development of interventional technology, as well as the development and update of devices such as stent-like devices, vascular implants, and intracapsular spoilers, WNBA can also be effectively treated. The present situation and progress of intravascular interventional therapy for WNBA are discussed by introducing new interventional techniques and equipment.
