摘要:目的 评估益生菌对老年慢性阻塞性肺疾病(COPD)患者的疗效及其对氨基酸代谢与血清免疫水平的影响。 方法 本研究招募了中国人民解放军联勤保障部队第九八〇医院2020年1月至2022年1月收治的80例老年COPD患者,采用随机数字表法将其分为常规治疗组和联合治疗组(在常规治疗基础上服用益生菌),每组40例,疗程均为2周。比较两组治疗前后改良版英国医学研究委员会呼吸困难问卷(mMRC)和COPD评估测试(CAT)评分,采用酶联免疫吸附试验测定两组治疗前后肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6和IL-8水平,采用液相色谱-质谱检测两组治疗前后的氨基酸代谢情况。 结果 治疗后,两组CAT、mMRC评分及TNF-α、IL-6、IL-8、色氨酸、酪氨酸水平均低于治疗前,亮氨酸、脯氨酸、丙氨酸和丝氨酸均高于治疗前,且联合治疗组CAT、mMRC评分及TNF-α、IL-6、IL-8、色氨酸、酪氨酸水平低于常规治疗组,亮氨酸、脯氨酸、丙氨酸和丝氨酸均高于常规治疗组,差异有统计学意义(P<0.05)。 结论 益生菌联合治疗可以显著改善老年COPD患者临床症状,对其免疫水平和氨基酸代谢有积极影响,可能是治疗老年COPD的有效策略。
Abstract:Objective To evaluate the effect of probiotics and its influence on serum immune level and amino acid metabolism in elderly patients with chronic obstructive pulmonary disease (COPD). Methods In this study, 80 elderly COPD patients admitted to the 980th Hospital of Chinese People’s Liberation Army Joint Logistic Support Force from January 2020 to January 2022 were recruited and divided into conventional treatment group and combined treatment group (taking probiotics in addition to conventional treatment) by random number table method, with 40 cases in each group. The course of treatment was two weeks. Modified Medical Research Council (mMRC) and COPD assessment test (CAT) scores were compared between the two groups before and after treatment. The levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-6, and IL-8 were measured by enzyme-linked immunosorbent assay. The amino acid metabolism of the two groups before and after treatment was detected by liquid chromatograph/mass spectrometer. Results After treatment, CAT, mMRC scores, and TNF-α, IL-6, IL-8, tryptophan, tyrosine levels in both groups were lower than those before treatment, while leucine, proline, alanine, and serine levels were higher than those before treatment, CAT, mMRC scores, and TNF-α, IL-6, IL-8, tryptophan, tyrosine levels in combined treatment group were lower than those in conventional treatment group, while leucine, proline, alanine, serine levels were higher than those in conventional treatment group, the differences were statistically significant (P<0.05). Conclusion Combined treatment with probiotics can significantly improve the clinical symptoms of elderly patients with COPD, and has positive effects on their immune level and amino acid metabolism, which may be an effective strategy for the treatment of elderly patients with COPD.
